The Flu Season is Coming: Brace Yourself for a Festive Challenge
Every winter, influenza makes its unwelcome appearance, and this year is no exception. But this season, Ireland is facing a particularly testing time with a surge in flu and cold cases. The flu has arrived a few weeks early, and it's not showing any signs of slowing down.
A Mutated Virus is to Blame:
The culprit? A mutated A(H3N2) virus, which is causing concern due to its early start and potential severity. In the past, seasons that began early have often been more severe, and this one is no exception. The A(H3N2) strain has taken the lead, replacing the previously dominant A(H1N1) strain from recent years.
Immunity is Low:
Here's where it gets tricky. Because this particular strain has been less prevalent in recent years, many people have reduced immunity against it. This means the virus can spread more easily, affecting a larger portion of the population.
A Virulent Strain with Serious Consequences:
The H3N2 strain is no ordinary flu. It's similar to the variant that caused the Hong Kong flu epidemic in 1968, and it's not just a respiratory illness. Professor Ronan Collins, a renowned expert, warns that it can significantly impact the heart and is a major risk factor for stroke. In some cases, it can even lead to delirium and cognitive issues, accelerating the onset of dementia.
The Silent Spreaders:
What's more, up to 75% of people infected with influenza may show no symptoms at all, yet they can still transmit the virus. This makes it even more challenging to contain the spread.
A Surge in Cases:
The numbers speak for themselves. In the week leading up to last Saturday, there were 2,944 confirmed influenza cases, with 657 patients requiring hospitalization. Alarmingly, the majority of these hospitalized patients were not vaccinated.
A Nasty Strain and Overcrowded Hospitals:
Dr. Conor Deasy, a leading voice in emergency medicine, describes the current strain as particularly nasty. This surge is putting immense pressure on hospitals that are already struggling with overcrowding. And with the flu season typically lasting between six to eight weeks, the situation is far from over.
Symptoms and Severity:
Influenza-like illness is characterized by a sudden onset of symptoms, including a temperature of 38°C or higher, along with at least two of the following: dry cough, headache, sore muscles, and a sore throat. The severity of this season is evident when compared to previous years. Last year saw 21,297 cases, while 2023 and 2022 had 11,751 and 10,392 cases, respectively. The Covid-19 pandemic year of 2021 saw a dramatic drop to just 39 cases due to lockdowns and preventative measures.
Flu Deaths and Excess Mortality:
This season has already claimed 13 lives due to influenza. During a typical flu season, 200-500 deaths are reported. However, in severe seasons, the death toll can be much higher. Research suggests that influenza may contribute to 400-800 excess deaths, and in particularly bad seasons, this number can reach 1,000-1,200. The elderly, those with chronic heart or lung disease, young children, individuals with cerebral palsy, and pregnant women are at higher risk.
The Impact on Healthcare:
The surge in flu cases is not an isolated issue. It's adding to the strain on emergency departments, leading to increased overcrowding. While RSV and Covid-19 cases are also circulating, their numbers are significantly lower compared to the flu. For instance, in the week ending last Saturday, there were 368 RSV and 176 Covid-19 confirmed cases, compared to 2,944 flu cases.
Geographical Distribution:
Since the start of this winter season, there have been 7,506 confirmed flu cases, with Dublin, Cork, Westmeath, Galway, and Wexford being the most affected areas. Laois, Mayo, and Monaghan have reported the fewest cases.
A Global Concern:
Ireland is not alone in this battle. Health authorities in England are reporting record-high case numbers for this time of year, and the NHS is bracing for the worst. Northern Ireland is also experiencing a rise in cases.
Nursing Homes Stay Prepared:
Nursing homes, aware of the situation, are well-prepared. With over 80% of residents vaccinated, they aim to maintain social connections without restrictions unless absolutely necessary.
Vaccination: A Crucial Step:
Doctors emphasize that vaccination is the best defense against the flu. Ideally, individuals should get vaccinated before the season starts, but there's still time to act. The HSE offers free flu vaccines to various age groups through GPs, pharmacies, and nursing homes.
Mixed Signals on Vaccination:
However, the HSE's target of 75% uptake among health staff has never been met, sending mixed messages to the public. In recent years, health staff vaccination rates have fluctuated, ranging from 18% to over 71%. For the 2024/25 season, the uptake was 45%. This is concerning, as healthcare workers are on the front line, and their protection is vital for patient safety.
Enhanced Vaccines: A Missed Opportunity?
Controversially, the HSE did not offer the enhanced flu vaccine for over 65s, citing cost-effectiveness concerns. Instead, a standard vaccine was provided. Some argue that this decision may have contributed to the current pressures on healthcare. The question remains: could the enhanced vaccine have alleviated the strain on hospitals, GPs, and patients?
Vaccine Efficacy: A Complex Issue:
Dr. Colm Henry, the HSE Chief Clinical Officer, assures that the offered vaccine is highly effective and complies with WHO recommendations. However, recent reports suggest that the current vaccines may not be as effective against the mutated strain. This has led to some skepticism, with people questioning the vaccine's efficacy. But doctors emphasize that the vaccine is working as expected, with UK data showing 70-75% effectiveness in preventing hospital admissions for 2-17-year-olds and 30-40% for adults.

With the flu likely to peak during Christmas week, hospitals may need to postpone more planned surgeries and procedures than usual. Flu outbreaks will need to be managed, potentially leading to restricted visiting policies, reminiscent of the Covid-19 pandemic. All signs point to a challenging season for patients, healthcare staff, and the entire health system.
